CarePlus Health Plans, Inc. offers a variety of Health Maintenance Organization (HMO) options, including standard HMOs, Dual Eligible Special Needs Plans (HMO D-SNPs), Chronic Condition Special Needs Plans (HMO C-SNPs), and Point-of-Service (HMO-POS) plans.

CarePlus Health Plans, Inc. is a Medicare Advantage provider serving select counties in Florida. This includes:

  • South Florida: Broward, Miami-Dade, Palm Beach
  • Tampa Area: Hillsborough, Pinellas, Pasco, Polk
  • Orlando Area: Orange, Osceola, Seminole, Lake, Marion, Sumter
  • Daytona Area: Flagler, Volusia
  • Treasure Coast: Brevard, Indian River
  • Jacksonville Area: Clay, Duval, St. Johns

CarePlus Medicare Advantage plans are available as HMOs, HMO D-SNPs, HMO C-SNPs, and HMO-POSs. All plans have a $0 premium.

All CarePlus Medicare Advantage plans are HMOs. These plans work with a network of CarePlus-approved providers.

Unless you’re experiencing a medical emergency, you must stay within your plan’s network to receive coverage.

HMO plans typically require a referral from your primary care provider (PCP) before they cover specialist visits. If you skip this step, your plan might not cover your service, even if the specialist is in your network.

CarePlus HMO

CarePlus offers five standard HMO plans for 2025, including:

  • CareFree Giveback HMO
  • Carefree Platinum Giveback HMO
  • CareAccess HMO
  • CareSalute HMO
  • CareOne Plus HMO

All plans include prescription drug (Part D) coverage except the CareAccess HMO.

CarePlus HMO D-SNP

If you’re dually eligible for both Medicare and Medicaid, a D-SNP plan can help coordinate your benefits.

CarePlus offers two D-SNP plans for 2025, CareNeeds Platinum D-SNP and CareNeeds Plus D-SNP. Both plans include Part D coverage.

CarePlus HMO C-SNP

C-SNPs are designed for people with certain chronic conditions. CarePlus offers three C-SNP plans for 2025, and each includes Part D coverage.

If you have diabetes, heart disorders, or chronic heart failure, you can enroll in CareComplete C-SNP or CareComplete Platinum C-SNP.

If you have a chronic lung disorder, you can enroll in CareBreeze Platinum C-SNP.

CarePlus HMO-POS

CarePlus three HMO-POS plans for 2025, including:

  • CareOne Plus HMO-POS
  • CareFree Platinum Giveback HMO-POS
  • CareSalute HMO-POS

All plans include Part D coverage except the CareSalute HMO-POS.

With an HMO-POS, you’ll be able to see some out-of-network providers in certain circumstances.

However, you’ll pay a significantly higher copay cost than you would if you stayed in your plan’s network.

CarePlus Medicare Advantage plans cover many of your healthcare needs.

You’re covered for the same basic services that Original Medicare provides, in addition to supplemental health and wellness benefits.

The following services are included with most CarePlus plans:

  • $0 copay for certain dental services, including dentures, extractions, and fillings
  • $0 copay for primary care, podiatry, acupuncture, and chiropractic visits
  • eyewear allowance
  • fitness program
  • hearing aid allowance
  • meal delivery program after overnight hospital stays
  • rewards program offering a $50 gift card after completing eligible health and wellness activities
  • wellness seminars

Some services are only available with certain plans, including:

  • annual allowance for dental, vision, and hearing services at participating providers
  • monthly allowance for groceries, rent, utilities, over-the-counter items, and other eligible expenses
  • Part D coverage, including a $0 copay for Tier 1 prescription drugs
